Pharmacists Leading Care Coordination

Today, one in five patients discharged from a hospital will be re-admitted within the following month, and of those re-admissions, 75% are preventable! Many of these re-admissions occur because there is no communication among providers.

Join us Nov. 18 to learn how your practice can break down the barriers that prevent seamless transitions among providers while improving the quality of your services. You’ll hear from pharmacists and physicians who provide transitional-care services for their patients. These leaders also advocate for pharmacist-led care coordination and communication - between pharmacies and clinics, primary caregivers and specialists, within various hospital departments, and upon discharge from hospital to home or skilled care.

Our program will feature speaker Nancy Skinner of the National Transitions of Care Coalition, plus area panelists Paul Iverson (Bemidji community pharmacist), Bob Beacher (President, Fairview Pharmacy Services), Bonnie Senst (Director, Allina Pharmacy Services), and Rahul Koranne, M.D. (Medical Director, HealthEast Bethesda Hospital)

We’ll meet in the exclusive Indoor Club room atop the new U of M football stadium, and you can even tour the stadium during the social hour! Event proceeds benefit the U of M College of Pharmacy's Center for Leading Healthcare Change. See you Nov. 18th!
