Who should register

This program is designed for pharmacists, pharmacy managers, educators, and students committed to developing a pharmaceutical care practice. Specifically, Levels 2 and 3 are for those interested in learning the details of the skills required and committed to applying the skills by providing and documenting pharmaceutical care with patients.


Learning Objectives

Level 2 objectives

  1. Understand the philosophy of practice that is the foundation of pharmaceutical care.
  2. Understand the responsibilities of the practitioner and the patient.
  3. Describe the therapeutic relationship and its purpose/function in the practice.
  4. Discuss the significance of the patient’s medication experience and how this information is used to guide drug therapy decisions.
  5. Explain the thought process involved in the Pharmacotherapy Workup and discuss its function in pharmaceutical care practice.
  6. Describe the steps involved in completing an assessment of a patient’s drug-related needs.
  7. Describe the common causes of each of the seven categories of drug therapy problems.
  8. Describe the components of a goal of therapy.
  9. Explain the function of goals of therapy in the care plan process.
  10. List the major functions involved in developing a care plan for a patient.
  11. Explain the responsibilities of the pharmaceutical care practitioner when performing a follow-up evaluation.
  12. Define the standard terms used to describe outcome status (stable, improved, etc.
  13. Describe the specific requirements for documenting patient care in practice.
  14. Explain why ethical considerations play such a significant role in pharmaceutical care practice.
  15. Discuss why all practitioners providing pharmaceutical care must:
    a) adhere to the same philosophy of practice,
    b) apply the same patient care process and
    c) present patient cases in the same manner

Level 3 objectives

  1. Apply the knowledge and skills learned in the first two levels of the certificate program to provide pharmaceutical care to 50 patients.
  2. Understand the responsibilities of the practitioner and the patient.
  3. Understand the standards of practice and document the care provided to your patients.

Continuing Education

Level 2 CE completion requirements:

  1. CE participants must be pharmacists.
  2. Participants must submit:
    1. Documentation for ten patients
    2. Four online journal reflections
    3. Documentation for three practitioner meetings
    4. A course evaluation

Level 3 CE completion requirements:

  1. CE participants must be pharmacists.
  2. Participants must submit:
    1. Documentation for fifty (50) patients
    2. A course evaluation.

Upon completion of these requirements, a statement of credit will be mailed to participants within four weeks.

Accreditation

Program Design

Level 2

Listen to Dr. Strand's brief course introduction

During the online course, learning materials are organized into six modules that will be released on a weekly basis. Participants can log in and access the available learning materials 24 hours/day-7 days/week, at their convenience during the course offering.

Participants will learn the skills and knowledge required to take responsibility for:

  • assessing each patient’s drug-related needs
  • developing care plans to meet these needs, and
  • completing a follow-up evaluation to determine the patient’s actual outcomes from the drug therapies.

Participants will be asked to complete the following activities:

  1. Schedule appointments with two new patients each week (initially patients can be friends/family; later, patients should be selected from place of employment).
  2. Document all patient encounters.
  3. Write weekly journal reflections on patient encounters.
  4. Participate in three practitioner meetings (face-to-face or in an online meeting room). Groups will be assigned once the course begins.

Patient documentation, journal reflections, and practitioner meeting documentation will be submitted online and assessed by course instructors.

Level 3

After completion of the Level 2 content, participants have a firm understanding of the skills required to practice pharmaceutical care. In Level 3 the emphasis shifts to accumulating experience. This includes solidifying and polishing one's skills and developing efficiency in the patient care process.

During this program, participants will:

  • Review interactive tutorials on entering patients in the Assurance Pharmaceutical Care System.™
  • Obtain a personal ID/password that provides access to the documentation system
  • Provide pharmaceutical care to forty-eight new patients and document a total of 50 encounters

Week 3

- Schedule two (2) new patient encounters and submit documentation.
- Complete reflection in online journal
Week 4
  • Module 4: Conducting an Assessment Interview
- Schedule two (2) new patient encounters and submit documentation.
- Attend second practitioner meeting.
Week 5
  • Module 5: Constructing a Care Plan

- Schedule two (2) new patient encounters and submit documentation.
- Complete reflection in online journal.
Week 6
  • Module 6: Conducting Follow-Up Evaluations
- Schedule two (2) follow-up patient encounters and submit documentation.
- Attend third practitioner meeting.
- Document previous ten (10) patients in Assurance Pharmaceutical Care System™
Week 7
Level 3
  • Module 1: Assurance Pharmaceutical Care System™
    Providing and Documenting Care
- Schedule eight (8) new patient encounters and doument in Assurance Pharmaceutical Care System™
Week 8
  • Providing and Documenting Care
- Schedule eight (8) new patient encounters and doument in Assurance Pharmaceutical Care System™

Prerequisites

Before enrolling, particpants should be sure that they:

  1. Have already completed the CE program, An Introduction to Pharmaceutical Care (Level I)
  2. Own a microphone for the practitioner meetings | View examples
  3. Own the textbook, Pharmaceutical Care Practice: The Clinician's Guide, second edition (should have been purchased for Level 1: An Introduction to Pharmaceutical Care.)

It is also strongly suggested that participants meet the following:

Tuition

Tuition for Level 2 is $725 and Level 3 is $245. For those registering for both levels together, a 20% discount is available, making the combined Level 2/3 tuition $776. No further discounts apply.
