Medication Therapy Management in Chronic Kidney Disease

Pharmacists frequently interface with Chronic Kidney Disease (CKD) patients because these patients are prescribed a multitude of medications. On average, a patient with end-stage kidney disease (ESKD) is prescribed 10-12 medications.
